About us:
Based in beautiful Whanganui Te Oranganui is a kaupapa Māori organisation who offers a range of health and wellbeing services for whānau in the Whanganui region and surrounding districts.
Te Waipuna Health is the General Practice arm with an enrolled population of 5,300 spread across two Practices. As a low-cost access health centre, we offer a newly refurbished Practice in a supportive and flexible environment.
About the role:
The role of Care Coordinator focuses primarily on supporting people and their whānau who are at the greatest risk of poor health outcomes, particularly Māori and Pasifika who require their care to be transferred between providers and across services including secondary care. This includes navigation and advocacy with relevant hauora services and education, housing, justice, cultural connection, spiritual and environment services where relevant.

You will have the ability to connect compassionately and collaboratively with whānau and health care professionals. You will have a commitment to improving health outcomes for whānau. Being flexible, adaptable and a critical thinker will be additional strengths as you’ll find that no two days are the same. You will have the ability to connect with different services across the rohe.
